MS HOPE Foundation provides financial assistance for MS-related expenses (see what we cover here). In order to apply for assistance you must complete our application form and submit the following required paperwork:
- A letter signed by your neurologist that states your MS diagnosis and includes the doctor’s contact information
- A copy of your (or your spouse’s; partner’s; or parent/guardian’s, if you’re under 18) two most recent pay stubs as proof of household income
- A copy of your driver’s license or state ID card
- A copy of your (or your spouse’s; partner’s; or parent/guardian’s, if you’re under 18) W-2’s from the past two years (if you do not receive W-2’s, please provide information that explains how you are currently supporting yourself)
- A copy of your (or your spouse’s; partner’s; or parent/guardian’s, if you’re under 18) W-2’s from the past two years (if you do not receive W-2’s, please provide information that explains how you are currently supporting yourself)
We recommend that you gather these documents before beginning the application process. Your application will not be considered until we’ve received all of your information.
We reserve the right to request additional documentation and to conduct background checks.
Please note: We DO NOT cover mortgage or rent payments. We ONLY pay providers directly; we DO NOT write personal checks under any circumstances.
